Der Sir,
I am hear trying to get my Bounty & Pension.  I have impleyed M B Hill Esgr to collect the above for me and he wishes a surguns certificate of Disabiloty.  Mager Jones of Newton University Hospt Balt. has lefs those fire I cant not get a certificate from him as he Discharged me.  Will you pleas make out a certificate of Disability for me as you as you are acquainted with the nature of my wounds it has heald up but still pains sevear at times. I can't not wish much on that account. pleas fill up the enclosed certificate and send it to Mr Wm B. Hill No 70 W. Fayette Street Baltimore Md and you will oblige me veary much.
