DIRECTIONS.--Here specify the details with regard to entries made, opposite headings specified in italics, as required in the Directions on the last page. Besides which, information on the following heads will be given either on this sheet or in an accompanying letter of transmittal. FEVERS.--Their character and symptoms; an outline of the plans of treatment found most efficient, with remarks on the location and sanitary condition of camps or quarters during the prevalence of these disorders. DIARRHEA AND DYSENTERY.--Grade and treatment, with remarks on the character of the ration, and the modes of cooking. SCORBUTIC DISEASES.--Character and symptoms, with observations on causation, and a statement of the means employed to procure exemption. RESPIRATORY DISEASES.--Symptoms, severity, and treatment, with remarks on the sheltering of the troops, and the atmospheric conditions. Similar remarks on other preventable diseases. IMPORTANT cases of every kind should be reported in full. Where post-mortem examinations have been made, accounts of the pathological results should be carefully prepared. I certify that the above report is correct and true to the best of my knowledge and belief. , Surgeon. [page 3 break] [page 4] DIRECTIONS TO BE FOLLOWED IN MAKING THE REPORT OF SICK AND WOUNDED. I.--TAKEN ON SICK REPORT DURING THE MONTH. I. In making out this part of the report, the number of cases of each disease which may have occurred during the month is to be entered in ordinary numerals opposite the name of the disease. This entry will always be made in the column headed "All other Cases," except when patients are received by transfer from some general hospital in which their treatment commenced, and in which it is to be presumed they have been already reported. In this case, the number of each disease so received by transfer will be entered in teh column headed "Cases from other Hospitals." II. In making out the tabular list, those cases only are to be entered which have been taken sick or received into the hospital during the month represented by the report. Cases remaining from previous months ARE NOT to be entered in the tabular list. ALL deaths occurring during the month are, however, to be entered in teh column of deaths, whether the patient was received during the current month or previously. III. Patients returning from furlough or desertion are not to be entered as new cases in the tabular list of diseases, but the number thus returned is to be entered in the summary under the head of "Returned from furlough or desertion." Patients absent on furlough are not to be included in the number reported as remaining. IV. The individual diseases specified in the tabular list are merely thos which experience has proved to be most frequent. Cases of any other diseases which may occur are to be entered under the head of "Other diseases," in the class and order to which they belong. V. Whenever cases are entered opposite any of the headings printed in italics, the precise nature of the affection so reported must be set forth in detail in the remarks on the third page of the report. II.--GENERAL SUMMARY. I. In the General Summary, the TOTAL under the head "Take sick or wounded during the month" must agree with the total number of cases of all kinds entered in the "Tabular list of diseases;" this total added to the "Total remaining last report," and the number "Returned from furlough and desertion," produces the aggregate, and must, of course, equal it. II. The number entered in the summary, under "Died," must equal the number specified by name in the subsequent part of the report adn the total "Deaths" in the tabular list; excepting only in the case of those KILLED IN BATTLE, who are to be entered, when positively ascertained, in the list of deaths by name, but not in the tabular list or general summary. III. The number entered in the summary, under "Transferred to the Veteran Reserve Corps," must be equal to the number specified by name in the list of transfers; and the number entered as "Discharged" must equal the number entered in the list of discharges. IV. The sum of the numbers entered under the several heads, "Returned to duty," "Sent to general hospital," "Furloughed," "Transferred to Veteran Reserve Corps," "Discharged ," "Deserted," and "Died," added to the total "Remaining," must equal the "Aggregate." V. The next division of the summary is intended to show the comparative amount of sickness among commissioned officers and enlisted men. Under this head, only the number taken sick during the month is to be entered, and not the "Aggregate." The sum of the cases and deaths reported under these heads must equal the total number of cases and deaths in the tabular list. VI. The "mean strength of the command" is to be obtained as follows: The strength present on each day of the month, obtained from the consolidated morning report of the command, is to be added together and divided by the number of days in the month. This is to be done for officers and enlisted men separately, and the two results added together for the total. The nearest whole numbers are to be used, and not fractions or decimals. The column headed "Mean strength of command" will not be filled up in case of general hospitals. VII. The average number on sick report, daily, will be obtained from the morning reports, by adding together in two separate columns the number remaining each day in hospital and in quarters, and dividing each total by the number of days in the month. This part of the summary is only intended for troops, and not for general hospitals. III.--LIST OF SOLDIERS TRANSFERRED TO THE VETERAN RESERVE CORPS, DISCHARGED ON SURGEON'S CERTIFICATES, AND DIED. I. The greatest care and accuracy should be used in making out this list and in stating the degree of disability, as the pension of disabled soldiers, or of the families of the dead, often depends on the records thus furnished. The correct spelling of names is of the utmost importance. NOTE.-- Attention is called to the following Circular: CIRCULAR No. 4. SURGEON GENERAL'S OFFICE, WASHINGTON, D. C., February 19, 1864. I. Separate reports of sick and wounded rebel prisoners of war, treated in U. S. General or Post Hospitals, will be made monthly to this office. II. Whenever white and colored troops are treated in the same hospital (either General or Post) separate reports of sick and wounded will be made, in order that the sickness and mortality rates of each race can be correctly ascertained. III. Monthly Sick Reports from regiments or detachments of colored troops will present on the first page of the report the number of sick of enlisted men only; the names, rank, and diseases of sick officers of these regiments will be palced under the head of Remarks.
